What Supplements Affect Tacrolimus Levels?

Tacrolimus is not a medication to casually mix with a new powder, gummy, tea, or “natural” capsule. If you are asking what supplements affect tacrolimus levels, the safest answer is that several can, and many others have not been studied well enough to assume they are harmless.

For transplant recipients and others taking tacrolimus, a level that is too low can raise the risk of rejection. A level that is too high can increase the risk of kidney injury, tremors, headaches, high blood pressure, high blood sugar, and other side effects. That narrow margin is why your transplant team asks about every prescription, over-the-counter medicine, herb, vitamin, and supplement.

This is not about being afraid of supplements. It is about being selective. A label that says “plant-based,” “wellness,” or “immune support” does not tell you whether it fits safely beside tacrolimus.

Why supplements can change tacrolimus levels

Tacrolimus is processed largely through enzymes and transport systems in the liver and intestines, especially CYP3A and P-glycoprotein. Some ingredients slow those systems down. Tacrolimus can then build up in the bloodstream. Other ingredients speed them up, allowing the body to clear tacrolimus faster and potentially lowering the amount available to protect a transplant.

The effect is not always predictable. Dose, product formulation, your genetics, liver function, other medications, and even when you take the product can all matter. A tea, extract, tincture, and concentrated capsule made from the same plant may not behave the same way.

Tacrolimus blood tests are usually trough levels, meaning the sample is timed before your next dose. A new supplement can make a level change look mysterious unless your care team knows it was added.

Supplements most likely to affect tacrolimus levels

St. John’s wort can lower tacrolimus levels

St. John’s wort is one of the clearest supplements to avoid with tacrolimus unless your transplant team has specifically directed otherwise. It can strongly induce the systems that metabolize tacrolimus, causing blood levels to fall. There are documented cases of transplant rejection associated with this interaction.

It may appear in products marketed for mood, stress, sleep, or menopause. Do not rely on the front label alone. Read the Supplement Facts panel and the full ingredient list, especially with blends.

CBD and cannabis-derived products may raise levels

CBD can inhibit enzymes involved in tacrolimus metabolism. That means oils, gummies, capsules, drinks, and some topical products with meaningful absorption may raise tacrolimus exposure. THC-containing products can also complicate medication management, particularly when products vary in dose or include CBD.

The issue is not limited to products bought at a dispensary. Hemp extracts sold as supplements may contain CBD, and labels are not always as precise as they should be. Bring the product or a clear photo of its label to your pharmacist or transplant coordinator before using it.

Berberine and schisandra deserve extra caution

Berberine is common in products promoted for blood sugar, cholesterol, metabolism, or gut health. It may inhibit pathways that help process tacrolimus and could increase levels. Human evidence is limited, but the potential is serious enough that self-starting it is not a good idea for someone on tacrolimus.

Schisandra, sometimes called magnolia vine, is another herbal ingredient with evidence of a significant interaction. It can increase tacrolimus concentrations and has been studied for that effect. It may be found in adaptogen, energy, stress, or traditional herbal formulas.

Curcumin, turmeric, and concentrated herbal blends are not automatic yeses

Cooking with ordinary amounts of turmeric is different from taking high-dose curcumin extracts, often combined with black pepper extract. Piperine, the active compound in black pepper extract, is used to increase absorption of other compounds and may also affect drug metabolism and transport.

There are reports and plausible mechanisms suggesting turmeric or curcumin products could increase tacrolimus exposure, particularly at high doses or in enhanced-absorption formulas. The evidence is less definitive than it is for St. John’s wort, but “not proven” is not the same as “safe to try.” Ask your transplant pharmacist before using it.

The same cautious approach applies to multi-herb formulas. Products marketed for detox, inflammation, weight loss, energy, menopause, immunity, or stress may combine several extracts with limited interaction data. When a label has 15 ingredients, it is harder for your team to identify the risk and harder for you to know what caused a changed level.

Foods and drinks that belong in the same conversation

Grapefruit is not a supplement, but it is too relevant to leave out. Grapefruit and grapefruit juice can raise tacrolimus levels by interfering with intestinal metabolism. Pomelo and Seville orange products may pose similar concerns. Marmalades, juices, flavored beverages, extracts, and citrus blends can be easy to miss.

Pomegranate has also been reported as a possible concern with tacrolimus. The evidence is less settled, but because tacrolimus has such a narrow safety range, many transplant programs advise avoiding pomegranate juice or discussing it first. This is a good example of why your own program’s guidance should lead the way.

What about common vitamins and supplements?

Not every supplement changes tacrolimus levels directly. But direct interaction data are only one part of the decision.

Vitamin D, magnesium, omega-3s, CoQ10, probiotics, and prebiotics are often discussed in transplant and kidney care. Whether they make sense depends on your lab work, diagnosis, medication list, dose, and the exact product. For example, vitamin D may be prescribed or recommended based on blood levels, while magnesium may be appropriate for some people but not others. Neither should be chosen just because it is broadly popular.

A probiotic may not be known to alter tacrolimus concentration, yet immune suppression changes the safety conversation. Rare infections from probiotic organisms have been reported in highly vulnerable patients. Your clinician may be comfortable with a specific product in your situation, or may advise against it during periods of higher risk.

Minerals can also affect how certain medicines are absorbed when taken together. Although this is not the best-established mechanism for tacrolimus itself, it is still wise to ask whether a mineral supplement should be separated from any part of your medication schedule. Do not change the timing of tacrolimus on your own. Consistency in how and when you take it matters.

A safer way to add any supplement while taking tacrolimus

Before you buy or restart a supplement, send your transplant coordinator, nephrologist, or transplant pharmacist the exact product name, a photo of the Supplement Facts panel, the full ingredient list, and the dose you plan to take. “It is just a natural sleep supplement” is not enough information to evaluate a product safely.

If your team approves it, ask whether you need a tacrolimus level checked after starting. In some cases, the best answer will be no. In others, your team may want closer monitoring, a specific dose limit, or an alternative product with fewer variables.

Use one change at a time. Starting a probiotic, a new protein powder, an herbal tea, and a multivitamin in the same week creates unnecessary confusion if your labs change or you feel unwell. Keep a current medication and supplement list on your phone, including occasional products such as sleep gummies, electrolyte powders, cold remedies, and “as needed” teas.

When to call your care team

Call your transplant team promptly if you accidentally take a product containing St. John’s wort, CBD, grapefruit extract, or another ingredient they have told you to avoid. Do not wait for your next routine appointment.

Also reach out if you develop new tremors, severe headaches, confusion, unusual weakness, vomiting, diarrhea that lasts more than a day, reduced urine output, or a sudden change in how you feel after adding a product. These symptoms do not automatically mean your tacrolimus level is high or low, but they deserve attention.
